Every time the government makes a change to a survey or a form — or introduces a new survey or form — you have the right to weigh in on that decision.

If you want to tell the government what you think about the changes they are making, follow these steps:

  1. Click on the name of the survey or form.

    The link will bring you to a listing in the "Federal Register" — a daily newsletter filled with the government's regulations, surveys, forms, public meetings, and other actions

  2. Skim the listing to find instructions on how to submit a comment.

    For most listings, this information will be found in a section titled "Addresses"

  3. Write in to tell the government what you think!

    Most listings allow you to either:

    • Send an email to the point of contact for that survey or form
    • Send your thoughts to the agency or the White House through the reginfo.gov website

2026-06-15 Private School Universe Survey (PSS) ED The PSS is designed to collect biennial data on the total number of private schools, teachers, and students; and to create an NCES universe frame of private schools that serve as a sampling frame for NCES surveys. 2026-06-16 Personal Responsibility Education Program (PREP) Performance Measures ACF The PREP program provides grants to states, tribes and tribal communities, and community organizations to support evidence-based programs to reduce teen pregnancy and sexually transmitted infections. The forms in this collection were used to assess overall program performance and to better understand the characteristics of youth involved in programming; youth sexual risk behaviors and behaviors related to adulthood preparation prior to program participation; and youth behavior intentions at program exit. ACF seems to be making significant changes to the collection that would result in 40% less questions, but they have not made any supplemental materials available describing the changes. 2026-06-22 Income Driven Repayment Plan Request ED This form is used by students to enroll in, recertify, or change their Income Driven Repayment (IDR) program. Under this listing, ED is implementing or formalizing several changes required by the OBBBA. One especially notable change to the form changes the default for students who no longer qualify for the IDR program in which they're currently enrolled - instead of placing such students into general forbearance while the student submits a new IDR application or changes to a different program they are eligible for, ED will place them into a standard repayment plan (which generally requires significantly higher monthly payments). 2026-06-29 Individual Complaint of Employment Discrimination Form DOT This form is used to file formal equal employment opportunity (EEO) discrimination complaints against DOT and, in turn, for DOT to process the complaints. The listing indicates that the form is being revised, but no supplemental materials are available describing the changes being made. Historically, this form has allowed respondents to indicate discrimination based on sexual orientation and gender identity, as well as race, disability, and other protected characteristics. 2026-06-29 Supplemental Nutrition Assistance Program's Quality Control Review Schedule USDA/FNS These forms are used by FNS to to collect both Quality Control (QC) data and case characteristics for SNAP and to serve as the comprehensive data entry form for SNAP QC reviews. Historically, these forms have collected information on race, sex, education level, employment status, and other characteristics of SNAP recipient households. 2026-06-30 Projects for Assistance in Transition From Homelessness (PATH) Program Annual Report SAMHSA The PATH Program Annual Report summarizes how states and local homelessness services providers use PATH funds and whether those uses align with the requirements of the Public Health Services Act - which requires providers not to discriminate on the basis of race, sex, national origin, or other protected characteristics. 2026-07-06 Addressing DEI Discrimination by Federal Contractors GSA This new form will collect information necessary for compliance with EO 14398, Addressing DEI Discrimination by Federal Contractors, which prohibits contractors from engaging in any racially discriminatory diversity, equity, and inclusion (DEI) activities.
