What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Key Information

Abstract

NIOSH is charged with developing reasonable estimates of the radiation doses incurred occupationally by U.S. nuclear weapons workers with cancer that are covered by the Energy Employees Occupational Illness Compensation Program Act of 2000. These radiation dose reconstructions are completed using methods promulgated by HHS under 42 CFR 82. An essential data source for completing the dose reconstructions is the information collected through structured telephone interviews conducted with claimants, who may be energy employees or surviving family members. In some cases, if necessary, co-workers of the employee are interviewed. These individuals may have unique knowledge of work conditions, radiation protection measures, and monitoring practices that are not available from other sources. At the completion of the dose reconstruction process, claimants review the estimated radiation dose, and signify that they have no more relevant information to offer by submitting a conclusion form (OCAS-1). This Extension requests OMB approval for an additional three years. There are no changes to the information collection instruments or burden on respondents.

[NIOSH] Reducing Fatigue Among Taxi/Rideshare Drivers

Key Information

Abstract

The goal of this study is to evaluate an online, interactive training on fatigue management tailored to taxi drivers and other for-hire rideshare drivers. The proposed study is a longitudinal, randomized design comparing two interventions and a control over 5 study periods. One group of 60 drivers will receive both interventions: fatigue training and daily feedback on estimated fatigue score. A second group of 60 drivers will receive only the fatigue training. A third group of 60 drivers will receive neither intervention. All participating drivers will take a self-administered questionnaire, wear a wrist actigraph to measure sleep quality, complete a sleep and activities diary, and take a sleep health and driving knowledge assessment. Drivers taking the training will be asked to complete an evaluation. Findings will be used to improve the training program for potential future dissemination. This change request updates documents to follow executive orders.

FERC-517, Safety of Water Power Projects and Project Works

Key Information

Abstract

FERC-517 includes Chapters 15 through 18 of engineering guideline, governing the safety of hydropower projects licensed by the Commission under the Federal Power Act. These actions are intended to promote the safe operation, effective maintenance, and efficient repair of licensed hydropower projects and project works to ensure the protection of life, health, and property in surrounding communities. Specifically, the Commission's actions will: incorporate two tiers of project safety inspections by independent consultants, codify existing guidance requiring certain licensees to develop an owner’s dam safety program and a public safety plan, update existing regulations related to public safety incident reporting, and make various minor revisions.The regulations in 18 CFR Part 12 and the Engineering Guidelines outline the provisions and reporting requirements for the evaluation of safety of water power projects and project works that apply to: (1) Any project licensed under Part I of the Federal Power Act; (2) Any unlicensed constructed project for which the Commission has determined that an application for license must be filed under Part I of the Act; and (3) Any project exempted from licensing under Part I of the Federal Power Act, pursuant to subparts J or K of 18 CFR Part 4, to the extent that the Commission has conditioned the exemption on compliance with any particular provisions of 18 CFR Part 12. Key Information

Abstract

On January 8, 2016, the Commission adopted a Report and Order, Second Report and Order, and Order on Reconsideration, FCC 16-1 (323/CORES Order and Reconsideration Order). The 323/CORES Order and Reconsideration Order adopted changes to the ownership reporting requirements that are intended to further improve the ownership data collection. The 323/CORES Order and Reconsideration Order adopts a Restricted Use FRN within CORES that individuals may use solely for the purpose of broadcast ownership report filings. In light of the Commission’s adoption of the Restricted Use FRN, the 323/CORES Order and Reconsideration Order eliminates the availability of the interim Special Use FRN for broadcast station ownership reports, except in very limited circumstances. The 323/CORES Order and Reconsideration Order prescribes revisions to Form 323-E that conform the reporting requirements for noncommercial educational (NCE) broadcast stations more closely to those for commercial stations, including revisions to collect race, gender, and ethnicity information for attributable interest holders; require that CORES FRNs or Restricted Use FRNs be used; and conform the biennial filing deadline for NCE broadcast ownership reports with the biennial filing deadline for commercial station ownership reports. The 323/CORES Order and Reconsideration Order also makes a number of other improvements to the forms that are intended to reduce the filing burdens on broadcasters, streamline the filing process, and increase the quality and usability of the data submitted to the Commission. These changes include extending the biennial filing deadline, reducing the number of filings required, improving the reporting of other broadcast interests, and additional improvements. Please se the justification for this non-substantive change request submission to the Office of Management and Budget for review and approval.

Federal Hotel and Motel Fire Safety Declaration Form

Key Information

Abstract

The Federal Hotel and Motel Fire Safety Declaration Form, FEMA Form FF-USFA-FY-21-112 (formerly 516-0-1), is used to collect basic information on life-safety systems related directly to fire-safety in hotels, motels, and similar places of accommodation applying for inclusion on the NML. In completing the form, applicants—property owners, lodging managers, or their delegates—provide basic information including identification of the owner(s) of the property, the business’s Employer Identification Number (EIN), and provisions for life-safety from fire. The form requests specific responses from applicants as to the installation of smoke alarms in all guestrooms of properties submitted for listing on the NML. In addition, applicants must indicate if the lodging establishment has an automatic fire sprinkler system where the building is four stories or higher. Once approved for the NML, the database record for each lodging establishment is assigned a unique identification number (FEMA ID). The information is collected and distributed through an on-line, electronic database accessed through the USFA website. Information submitted voluntarily by lodging establishments is reviewed and if the criteria meet the requirements of Public Law 101-391, they are given a FEMA ID number and listed in the NML database. Federal travelers and the public seeking public accommodation with provisions for life-safety from fire access the NML through the on-line database and identify lodging establishments in the area to which they are traveling.

The Volunteer Income Tax Assistance (VITA) program developed the Forms 13614-C and 13614-NR that contain a standardized list of required intake questions to guide volunteers in asking taxpayers basic questions about themselves. The forms are effective tools to ensure VITA volunteers consistently collect personal information from each taxpayer to assure the tax returns are prepared accurately and avoid erroneous returns. These forms are critical to continued improvements in the accuracy of volunteer-prepared returns for taxpayers utilizing the VITA program. Form 13614-C is used to assist VITA volunteers in preparing individual tax returns for citizens or residents of the United States. The IRS also provides multiple translations of Form 13614-C. Form 13614-NR is used to assist VITA volunteers in preparing individual tax returns for nonresident aliens.

Fatality Analysis Reporting System (FARS)

Key Information

Abstract

The FARS is a voluntary information collection of fatal motor vehicle traffic crashes. The FARS is in its forty-sixth year of operation and is a census of all defined crashes involving fatalities in the country. The FARS collects data from all 50 States, the District of Columbia, and Puerto Rico under cooperative agreements. State employees extract and transcribe information from existing State files including police crash reports as well as driver license, vehicle registration, highway department, and vital statistics files. NHTSA aggregates the data for research and analysis in support of motor vehicle regulations and highway safety programs. This supports NHTSA’s mission to save lives, prevent injuries, and reduce economic losses resulting from motor vehicle crashes by providing the agency vital information about fatal crashes. The aggregated data comprises a national database that is NHTSA’s and many States’ principal means of tracking trends in fatalities and quantifying problems or potential problems in highway safety. The FARS data are used extensively by all the NHTSA program and research offices, other DOT modes, States, and local jurisdictions. The highway research community uses the FARS data for trend analysis, problem identification, and program evaluation. Congress uses the FARS data for making decisions concerning safety programs. The FARS data are also available upon request to anyone interested in highway safety. The annual burden has been adjusted from 106,244 to 107,209 hours (an increase of 965 hours) costs have decreased from $100,000 to $0 (a decrease of $100,000). The adjustment in burden hours is due to the increase in the complexity of coding the FARS cases and an increase in the number of fatal crashes across most States. The increase also accounts for the time to process the non-traffic fatalities for NTS. Furthermore, while time for manually inputting data has decreased with States implementing systems to electronically transfer police report data that prepopulate NHTSA’s data systems, including FARS, the overall burden increased because, over the past two years, there has been an increase in staff turnover at the State level, adding an increase in administrative hours, training, and coding assistance to continue operations. The decrease in costs is a result of removing labor costs associated with labor hours that were incorrectly included in our last ICR.

Abstract

The National Recreational Boating Safety Survey aims to estimate the total population of boaters in the United States and by state and collects attitudinal data on public safety interventions such as safety equipment and boating safety courses. The survey also estimates the total boat population of paddlecraft (canoes, kayaks, standup paddleboards) that are not required to be registered with a state authority like a Department of Motor Vehicles. Since the 2018 NRBSS was conducted, canoe and kayak fatalities have risen from 21% to 26% of total fatal incidents, accounting for nearly 150 deaths in 2023.

Abstract

Under the Youth Conservation Corps Act of August 13, 1970, as amended (U.S. 1701–1706), the U.S. Department of Interior and the U.S. Department of Agriculture-U.S. Forest Service provide seasonal employment for eligible youth 15 through 18 years old. The Youth Conservation Corps stresses three important objectives: 1. Accomplish needed conservation work on public lands; 2. Provide gainful employment for 15 to 18 year-old males and females from all social, economic, ethnic, and racial backgrounds; and 3. Foster, on the part of the 15 through 18 year-old youth, an understanding and appreciation of the Nation’s natural resources and heritage. Youths seeking training and employment with the Youth Conservation Corps must complete the following new common forms included in this emergency clearance request: DI-4014, “Youth Conservation Corps Application” and DI-4015, “Youth Conservation Corps Medical History.” The applicants’ parents or guardians must sign both forms. The application and medical history forms are evaluated by participating agencies to determine the eligibility of each youth for employment with the Youth Conservation Corps. Potential and actual agencies that may use the common forms included in this collection include: • U.S. Fish and Wildlife Service (Interior); • National Park Service (Interior); and, • Other Federal Departments and Agencies such as the U.S. Forest Service (U.S. Department of Agriculture).

Key Information

Abstract

The Public Lands Corps is a work and education program involving the nation’s land management agencies, conservation and service corps, and environmental organizations that contribute to the rehabilitation, restoration, and repair of public lands resources and infrastructures. Public Lands Corps projects provide opportunities for community and national public service, work experience and training for young people who are unemployed or underemployed persons, students, recent graduates, and others with an interest in natural and cultural resources careers. The Agency has developed a tracking sheet (FS-1800-0026, Workforce Development Participant Tracking Form) to support the effective management of the Public Lands Corps Program. The utilization of the Workforce Development Participant Tracking Form will assist the Agency in ensuring qualified program participants lawfully receive a certificate of non-competitive hiring eligibility,which can be used to apply to Forest Service merit announcements on USAJobs.

Holding Company Report of Insured Depository Institutions' Section 23A Transactions with Affiliates

Key Information

Abstract

This reporting form collects information on covered transactions between an insured depository institution and its affiliates that are subject to the quantitative limits and other requirements of section 23A of the Federal Reserve Act (12 U.S.C. § 371c) and the Board’s Regulation W - Transactions Between Member Banks and Their Affiliates (12 CFR Part 223). The FR Y-8 is filed quarterly by all U.S. top-tier bank holding companies (BHCs), intermediate holding companies (IHCs), and savings and loan holding companies (SLHCs) that control an insured depository institution that has had any section 23A transactions with affiliates in a given quarter. It is also filed quarterly by foreign banking organizations (FBOs, and together with top-tier U.S. BHCs, SLHCs, and IHCs (holding companies)) that directly own or control such a U.S. subsidiary insured depository institution. If an FBO indirectly controls such a U.S. insured depository institution through a domestic U.S. holding company, the domestic U.S. holding company must file the FR Y-8. A respondent must file a separate FR Y-8 report for each U.S. insured depository institution it controls that has had section 23A transactions with affiliates. If an insured depository institution does not conduct any section 23A covered transactions with an affiliate in a particular quarter, its parent holding company is not required to submit an FR Y-8 report for that quarter with respect to that subsidiary.

Maritime Administration (MARAD) Jones Act Vessel Availability Determinations

Key Information

Abstract

Pursuant to 46 U.S.C. § 501(b), the Maritime Administrator is required to make determinations of the availability of qualified United States flag capacity to carry coastwise cargo in connection with all requests for waivers of the Jones Act (46 U.S.C. § 55102). Based upon determinations made by the Maritime Administrator, and in consultation with the Maritime Administrator, the head of the Department of Homeland Security acting through Customs and Border Protection can waive compliance with the Jones Act. The collected information will be used by the Maritime Administration (MARAD), Office of Cargo and Commercial Sealift, to fulfill its statutory obligation in determining U.S. flag availability. The respondents are coastwise qualified vessel owners, operators, charterers, brokers and representatives. The collection of information is voluntary.
