Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

Part 20 of title 10 of the Code of Federal Regulations, ‘‘Standards for Protection Against Radiation’’ establishes standards for protection against ionizing radiation resulting from activities conducted under licenses issued by the NRC and by Agreement States. These standards require the establishment of radiation protection programs, maintenance of radiation protection programs, maintenance of radiation records, recording of radiation received by workers, reporting of incidents which could cause exposure to radiation, submittal of an annual report to NRC and to Agreement States of the results of individual monitoring, and submittal of license termination information. These mandatory requirements are needed to protect occupationally exposed individuals from undue risks of excessive exposure to ionizing radiation and to protect the health and safety of the public.

Abstract

When a member of the public requests a Social Security record, SSA needs the name and address of the requestor as well as a description of the requested record to process the request. SSA uses the information the respondent provides on Form SSA-711, or via an Internet request through SSA’s electronic Freedom of Information Act Xpress (FOIAXpress) website, to: (1) verify the wage earner is deceased; and (2) access the correct Social Security record. Respondents learn of the possibility for these requests via Agency mailers; multi-media commercials and advertising; SSA’s general website; a link from the mySocial Security landing page; as well as from other individuals. SSA is constantly reviewing our outreach tactics to assure maximum exposure and accessibility to all individuals who need this service. Our website and mailers list the information respondents need to apply for requests. Respondents are members of the public requesting deceased individuals’ Social Security Records.

Abstract

The Early Childhood Longitudinal Study (ECLS) program, conducted by the National Center for Education Statistics (NCES) within the Institute of Education Sciences (IES) of the U.S. Department of Education (ED), draws together information from multiple sources to provide rich, descriptive data on child development, early learning, and school progress. The ECLS program studies deliver national data on childrens status at birth and at various points thereafter; childrens transitions to nonparental care, early care and education programs, and school; and childrens experiences and growth through the elementary grades. The Early Childhood Longitudinal Study, Kindergarten Class of 2023-24 (ECLS-K:2024) is the fourth cohort in the series of early childhood longitudinal studies. The study will advance research in child development and early learning by providing a detailed and comprehensive source of current information on childrens early learning and development, transitions into kindergarten and beyond, and progress through school. The ECLS-K:2024 will provide data about the population of children in kindergarten during the 2023-24 school year. The request to conduct the first three national data collection rounds for the ECLS-K:2024 was approved in April 2023 (OMB# 1850-0750 v26). Revisions to procedures and materials for the first two rounds of data collection were submitted and approved in three subsequent revisions requests (OMB# 1850-0750 v27 was approved in July 2023; OMB# 1850-0750 v29 was approved in February 2024; and OMB#1850-0750 v30 was approved in July 2024). The ECLS-K:2024 fall kindergarten data collection was conducted August 2023-January 2024 and the spring kindergarten round was conducted March-July 2024. These two kindergarten data collections will be followed by a spring (March-July 2025) first-grade round. Each of these rounds of data collection involves advance school contacts, for example to conduct student sampling activities, collect teacher and school information, and locate families whose children may have moved schools. Future OMB packages are planned for the third-grade pilot test (March-July 2026), as well as for the future currently-planned round, that is the national spring (March-July 2027) third-grade round. This current revision request (accompanied by 30 days of public comment) is to update study respondent materials, surveys, and website designs that will be used in the spring 2025 first-grade data collection activities. Many of the revisions in this package were based on analyses of the fall 2022 field test data (OMB# 1850-0750 v25) and experiences in the field from the national kindergarten rounds of collection, as well as additional discussions with design experts, all of which informed changes to the design of the surveys and assessments. Changes have been made to respondent materials to remove references to the fifth-grade round, as this collection is no longer planned. Revisions to the study instruments, the respondent materials, and websites are limited to changes to the spring first-grade materials. The current revision request contains updates to the school staff and parent survey instruments made as the specifications were programmed. This revision request also contains the specifications for the Spanish and Mandarin translations of some parent materials. Finally, this request revises the race/ethnicity items in the web and hard-copy first-grade survey instruments in line with the new federal statistical standard released on 3/28/24 (Statistical Policy Directive No. 15: Standards for Maintaining, Collecting, and Presenting Federal Data on Race and Ethnicity (SPD 15)). The requested changes in this revision request reflect a slight increase in the cost to the federal government, but do not change the estimated respondent burden for conducting this study.

Abstract

VA is keeping this information collection open for resubmissions regarding the loan modification options available to servicers that do not require VA’s prior approval to include the COVID-19 Refund Modification, to assist certain COVID-impacted veterans as they exit a COVID-19 forbearance. Under 38 U.S.C. 3720(a)(2), Congress has provided the Secretary with discretion “[n]otwithstanding the provisions of any other law” to set the terms and conditions to which the Secretary will consent to loan modifications. Additionally, while VA has outlined in regulation at 38 CFR 36.4315(a) the terms of loan modifications that do not require prior VA approval, VA may waive a regulatory requirement if VA finds the interest of the Government are not adversely affected and such waiver would relieve undue prejudice to a debtor, holder, or other person without impairing the vest rights of any person affected. 38 CFR 36.4338(a). Under the COVID-19 Refund Modification, servicers will be able to combine a partial VA purchase (refund) that includes arrearages and, if needed, outstanding principal amounts (hereinafter known as a COVID-19 Refund) with a modification of the guaranteed loan terms to achieve an affordable loan payment for the veteran. Servicers who offer the COVID-19 Refund Modification will be required to originate a new loan repayable to VA and provide documentation to VA. This collection of information is necessary to ensure that the COVID-19 Refund is appropriately established as a loan repayable to VA. Specifically, the servicer is required to prepare a note and security instrument in favor of “the Secretary of Veterans Affairs, an Officer of the United States.” VA requires that the note be consistent with the terms described in the Circular and include all borrowers who are obligated on the guaranteed loan. The security instrument is required to include all non-borrowers who have an interest in the property securing the guaranteed loan. The servicer is required to submit to the Secretary the request for COVID-19 refund within timelines prescribed in the Circular. VA also requires the servicer to provide VA with the original note no later than 120 days after the veteran exits forbearance; the servicer must also provide VA with the original security instrument and evidence of recordation not later than 180 days following the date the security instrument is executed. If the recording authority causes a delay, VA allows the servicer to request an extension of time, in writing, from VA. Finally, the servicer is required to report information related to the COVID-19 Refund Modification event to VA electronically; this information, however, has been included as a revision to an already approved information collection for VA’s electronic loan servicing system (OMB control number 2900-0021).

Abstract

Internal Revenue Service regulations require employers to provide wage and tax data to SSA using Form W-2 or its electronic equivalent. As part of this process, the employer must furnish the employee's name and Social Security number (SSN). In addition, the employee's name and SSN must match SSA's records for SSA to post earnings to the employee's earnings record, which SSA maintains. SSA offers the Social Security Number Verification Systems (SSNVS), which allows employers to verify that the reported names and SSNs of their employees match SSA’s records. SSNVS is a cost-free, voluntary method for employers to verify employee information via the Internet. SSA annotates data an employer supplies to SSA for verification that does not match SSA’s records with a no match indicator and returns it to the employer. SSA retains an audit trail of all supplied data. In addition, SSA uses the data collected as part of its continual evaluation of the SSNVS process. SSA does not use the information provided for any other purposes. The respondents are employers who need to verify SSN data using SSA's records.

Abstract

DHIS collects on an annual basis, statistically valid data on the amount, distribution, and effects of illness and disability in the population and on the utilization of health care services for such conditions. NHIS data are used widely throughout the Department of Health and Human Services (DHHS) to monitor trends in illness and disability and to track progress toward achieving many of the health objectives for the nation. This Revision includes updated Survey information and burden edits. The current design of the NHIS questionnaire, implemented in 2019, features a rotational schedule consisting of annual core, rotating core, emerging content, and sponsored content modules. Methylene Chloride Standard (29 CFR 1910.1052)

Abstract

The standard requires employers to monitor employee exposure to methylene chloride (MC), to provide medical consultation and examinations, to train employees about the hazards of MC in their working areas, and to establish and maintain records of employee exposure to MC. These records will be used by employers, employees, physicians and the Government to ensure that employees are not being harmed by exposure to MC. Appendix B to section 1910.1052 contains medical surveillance questionnaire forms for workers that may be exposed to methylene chloride. Based on the animal evidence and three epidemiologic studies previously mentioned, OSHA concludes that Methylene Chloride is a suspect human carcinogen. The medical surveillance program is designed to observe exposed workers on a regular basis.

Broadcast Incubator Program

Abstract

On August 3, 2018, the Commission released a Report and Order (Order), Rules and Policies to Promote New Entry and Ownership Diversity in the Broadcasting Services, FCC 18-114, in MB Docket No. 18-289, establishing the requirements that will govern the incubator program that the Commission previously decided to adopt to support the entry of new and diverse voices into the radio broadcast industry. The Commission recognized the need for more innovative approaches to encourage access to capital, as well as technical, operational, and management training for use by new entrants and small businesses, that without assistance, would not be able to own broadcast stations. The incubator program is designed for small businesses, struggling station owners, and new entrants that do not have any other means to access the financial assistance and operational support necessary for success in the broadcast industry. The goal is the pairing of these small aspiring, or struggling, broadcast station owners with established broadcasters. These incubation relationships will provide new entrants and struggling small broadcasters access to the financing, mentoring, and industry connections that are necessary for success in the industry, but to date have been unavailable to many. In return for successfully incubating a small aspiring, or struggling, broadcast station owner as part of the incubator program, an incumbent broadcaster will be eligible to receive a waiver of the Local Radio Ownership Rule following the conclusion of a successful qualifying incubation relationship.
