Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The Federal Hazardous Substance Act (Pub. L. 86-613) and Hazardous Material Transportation Act (Pub. L. 93-633) prescribe standards for packaging of hazardous substances. To meet the requirements of the Acts, the General Services Administration Regulation prescribes provision 552.223-72, Hazardous Material Information, to be inserted in solicitations that provides for delivery of hazardous materials on an f.o.b. origin basis. The provision requires the contractor to identify for each National Stock Number (NSN) the Department of Transportation (DOT) Shipping Name, DOT Hazards Class, and whether the item requires a DOT label.

Key Information

Abstract

The standard requires employers to monitor employee exposure to methylene chloride (MC), to provide medical consultation and examinations, to train employees about the hazards of MC in their working areas, and to establish and maintain records of employee exposure to MC. These records will be used by employers, employees, physicians and the Government to ensure that employees are not being harmed by exposure to MC. Appendix B to section 1910.1052 contains medical surveillance questionnaire forms for workers that may be exposed to methylene chloride. Based on the animal evidence and three epidemiologic studies previously mentioned, OSHA concludes that Methylene Chloride is a suspect human carcinogen. The medical surveillance program is designed to observe exposed workers on a regular basis.

Abstract

On August 3, 2018, the Commission released a Report and Order (Order), Rules and Policies to Promote New Entry and Ownership Diversity in the Broadcasting Services, FCC 18-114, in MB Docket No. 18-289, establishing the requirements that will govern the incubator program that the Commission previously decided to adopt to support the entry of new and diverse voices into the radio broadcast industry. The Commission recognized the need for more innovative approaches to encourage access to capital, as well as technical, operational, and management training for use by new entrants and small businesses, that without assistance, would not be able to own broadcast stations. The incubator program is designed for small businesses, struggling station owners, and new entrants that do not have any other means to access the financial assistance and operational support necessary for success in the broadcast industry. The goal is the pairing of these small aspiring, or struggling, broadcast station owners with established broadcasters. These incubation relationships will provide new entrants and struggling small broadcasters access to the financing, mentoring, and industry connections that are necessary for success in the industry, but to date have been unavailable to many. In return for successfully incubating a small aspiring, or struggling, broadcast station owner as part of the incubator program, an incumbent broadcaster will be eligible to receive a waiver of the Local Radio Ownership Rule following the conclusion of a successful qualifying incubation relationship.

Abstract

The Federal Trade Commission (“FTC” or “Commission”) seeks Office of Management and Budget (“OMB”) clearance for revised reporting requirements under its Hart-Scott-Rodino Antitrust Improvements Act Rules and corresponding Premerger Notification and Report Form for Certain Mergers and Acquisitions. On June 29, 2023, the FTC submitted a Notice of Proposed Rulemaking (“NPRM”) and an accompanying Supporting Statement to the OMB for review under the Paperwork Reduction Act (“PRA”). On July 7, 2023, OMB directed the Commission to resubmit its request when the proposed rule was finalized. The Final Rule was promulgated on November 12, 2024, and will be effective on February 10, 2025. The Commission is now submitting the Final Rule amendments and a Supplemental Supporting Statement to OMB.

Assessment Rate Adjustment Guidelines for Large and Highly Complex Institutions

Key Information

Abstract

The guidelines determine how adjustments can be made to the total scores that are used in calculating the deposit insurance assessment rates of large and highly complex insured institutions, under the methodology of the Amended Assessment Regulations. These amended regulations eliminated risk categories for large institutions and combined CAMELS ratings and forward-looking financial measures into one of two scorecards, one for highly-complex institutions and another for all other large institutions. Each of the two scorecards produces two scores-a performance score and a loss severity score-that are combined into a total score, which cannot be greater than 90 or less than 30. The FDIC can adjust a bank's total score up or down by no more than 15 points, but the resulting score cannot be greater than 90 or less than 30. The score is then converted to an initial base assessment rate, which, after application of other possible adjustments, results in a total assessment rate. The total assessment rate is multiplied by the bank's assessment base to calculate the amount of its assessment obligation.

Abstract

The High School and Beyond Longitudinal Study of 2022 (HS&B:22) is the sixth in a series of longitudinal studies at the high school level conducted by the National Center for Education Statistics (NCES), within the Institute of Education Sciences (IES) of the U.S. Department of Education. HS&B:22 is following a nationally representative sample of ninth grade students from the start of high school in the fall of 2022 to the spring of 2026 when most will be in twelfth grade. The sample will be freshened in 2026 to create a nationally representative sample of twelfth-grade students. A high school transcript collection and additional follow-up data collections beyond high school are also planned. A field test was conducted in fall 2019 and the first follow-up field test (F1FT) is planned for spring 2024 in preparation for the spring 2026 first follow-up full-scale study (F1FS).This submission is to request approval to conduct the HS&B:22 F1FT collection in the spring of 2024. OMB provided approval for F1FT sampling, tracking, and recruitment in March 2021 (OMB# 1850-0944 v.9). Part A of this submission presents information on the basic design of HS&B:22. Part B discusses the statistical methods employed. Part C presents justification for the questionnaire content. Appendix A provides the communication materials to be used during state, school district, school, student, and parent F1FT recruitment and data collection activities. Appendix B provides the first follow-up field test data collection instruments.

This is a request for OMB to approve the revised information collection, Cancer Therapy Evaluation Program (CTEP) Support Contracts Forms and Survey (OMB control number #0925-0753, expiration 3/31/2026), for three years. This revision removes one form, adds seven new forms, and revises five forms. The National Cancer Institute (NCI) CTEP and the Division of Cancer Prevention (DCP) fund an extensive national program of cancer research, sponsoring clinical trials in cancer prevention, symptom management, and treatment for qualified clinical investigators. As part of this effort, CTEP implements programs to register clinical site investigators and clinical site staff and to oversee the conduct of research at the clinical sites. CTEP and DCP also oversee two support programs, the NCI Central Institutional Review Board (CIRB) and the Cancer Trial Support Unit (CTSU). The combined systems and processes for initiating and managing clinical trials are termed the Clinical Oncology Research Enterprise (CORE) and represent an integrated set of information systems and processes that support investigator registration, trial oversight, patient enrollment, and clinical data collection. The information collected is required to ensure compliance with applicable federal regulations governing the conduct of human subjects research (45 CFR 46 and 21 CRF 50), and when CTEP acts as the Investigational New Drug (IND) holder (Food and Drug Administration (FDA) regulations pertaining to the sponsor of clinical trials and the selection of qualified investigators under 21 CRF 312.53). Survey collections assess satisfaction and provide feedback to guide improvements with processes and technology.

The PACE Organization Application Process in 42 CFR Part 460 (CMS-10631)

Key Information

Abstract

Initial application requirements for the PACE program are currently set forth in 42 CFR 460.12 and in the PACE Manual, Ch. 17. Until recently, the submission of initial and SAE PACE applications and supporting information was in paper format. These applications are often hundreds of pages long, expensive to reproduce and transmit, and administratively inefficient, as staff reviewing different parts of the application are located in different physical locations and must receive hard copies of the material. However, beginning in 2016, initial PACE applications are being submitted via a new automated, electronic submission process. As with initial applications, an application also must be submitted for a PO that seeks to expand its service area and/or add a new service site, and with OMB approval, an automated application process will now also be required of PACE organizations submitting service area expansion applications. In pursuing this new information collection package, we are extracting this component only from the existing OMB approval for the PACE program (CMS-R-244, OMB Control Number 0938-0790).

The FDIC regulations modernized the process of determining the insurance status of each depositor in the event of a depository institution failure. The regulations enable operations of a large insured depository institution to continue functioning on the day following failure, support the FDIC's efforts to fulfill its legal mandates regarding the resolution of failed insured deposit institutions, and apply to the largest institutions only ($2 billion in domestic deposits or more). More specifically, the regulations require the largest insured depository institutions to adopt mechanisms that would, in the event of the institution's failure, (1) provide the FDIC with standard deposit account and customer information, and (2) allow the FDIC to place and release holds on liability accounts, including deposits.

Abstract

The Commission requires all incumbent local exchange carriers (ILECs) to file shapfile maps of their service territories in a state (study area). State commissions can also submit these data voluntarily for ILECs in their state. Shapefiles are a commonly used, digitized, geographic information system (GIS) format. Accurate and accessible maps are essential to the legitimate distribution of universal service support to rural, high cost carriers. After the shapefiles are uploaded into a web interface provided by the Commission, each ILEC (or state) must certify the accuracy of its study area maps. Filers must also submit updated shapefile maps if their study area boundaries change, and must recertify the accuracy of the map every two years.

Rule 12d2-1, which was adopted in 1935, requires that national securities exchanges send written notice to the Commission of all trading suspensions. NCEE System Clearance For Design and Field Studies 2023-2026

Abstract

This is a request for a 3-year generic clearance for the National Center for Education Evaluation (NCEE) that will allow it to collect preliminary or exploratory information to aid in study design. The procedures expected to be used include but are not limited to exploratory surveys and interviews, focus groups, cognitive laboratory activities, pilot testing versions of an intervention or data collection approach, small-scale experiments that explore questionnaire design, incentives, or mode, and usability testing. NCEE is requesting an adjustment in the respondent burden hours for the first year, 2023-2024, to allow an additional information collection that would increase the cumulative total from 3,000 to 3,500 hours.

Administration for Children and Families Uniform Project Description (UPD)

Abstract

The Uniform Project Description (UPD) is standard language and sections available for use by Administration for Children and Families (ACF) program offices to solicit the required project description and project budget information from applicants for specific funding opportunities. Applications are required for project grants and cooperative agreements as prescribed by HHS regulations 45 CFR § 75.203. The UPD provides a selection of text options for the program offices to choose from, which are used to communicate the application requirements for a specific grant or cooperative agreement notice of funding opportunity (NOFO), as required by 45 C.F.R. § 75.203, and Appendix I to Part 75 – Full Text of Notice of Funding Opportunity. The UPD is fundamental to ACF’s competitive award process. It ensures the provision of adequate information to support award decisions. The ability to select the appropriate standard language for a given NOFO reduces the burden of application preparation, by eliminating unnecessary portions of the application for a given announcement. In addition, it provides for consistency and objectivity in the application and review processes required by 45 C.F.R. § 75.204. In March 2023, ACF obtained approval for the current ACF Uniform Project Description (UPD) (OMB expiration date: March 31, 2026). A subsequent nonsubstantive change request was approved in October 2023 to allow for an alternate, simplified version of the UPD to be piloted with 25 NOFOs. The pilot effort, which was part of the HHS SimplerGrants, SimplerNOFOs Initiative, was successful and ACF would like to expand use of the simplified version in 2025.

Rule 17f-2(a) requires that all securities personnel be fingerprinted.
