What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

Abstract

The purpose of this new information collection request for the approval of model documents for the Medicare Prescription Drug Program (Part D) is to meet the requirements of Section 11202 of the Inflation Reduction Act of 2022, Public L. 117-169 (IRA), which established the Medicare Prescription Payment Plan Program. Under this program, MA Organizations offering Part D coverage and Part D sponsors are required to offer enrollees the option to pay their Part D cost sharing in monthly amounts spread out over the plan year based on the formula described in section 1860D-2(b)(2)(E)(iv) of the Act. There are several education and outreach requirements for plan sponsors related to implementation of this program. The proposed documents in this information collection are designed to assist Part D plan sponsors and MA organizations in complying with Sections 1860D-2(b)(2)(E)(v)(II) – (IV) of the Social Security Act (the Act), which set forth the education and outreach responsibilities for Part D sponsors and MA organizations for this program.CMS has developed the six materials in the attached package as model notices to provide standardized and consistent language for potential and active program participants, regardless of which Part D plan they may be enrolled in. CMS will require Part D plans to disseminate these notices, as appropriate, to Part D enrollees to fulfill the requirements of the Sections 1860D-2(b)(2)(E)(v)(II) – (IV) of the Act. The six model notices and their related burden are accounted for in this PRA package. By providing these model notices, CMS is providing standardized messaging for all Part D beneficiaries and reducing administrative burden on Part D sponsors and MA organizations.

Abstract

Section 214 of the Communications Act of 1934, as amended, requires that the FCC review the establishment, acquisition, operation, and extension of lines, and the discontinuance of service by interstate common carriers. This OMB collection pertains primarily to section 63.71 and other related sections of the Commission's rules, which govern the application process for receiving authority for discontinuance, impairment or reduction in service. The discontinuance obligations that apply to domestic non-dominant telecommunications carriers have been extended to providers of interconnected VoIP service. In July, 2016, the Commission revised its discontinuance procedures to require that technology transition discontinuance applicants seeking streamlined treatment meet the "adequate replacement test." In June 2018, the Commission further modified the rules applicable to section 214(a) discontinuance applications. First, all carriers, whether dominant or non-dominant, that seek approval to grandfather data services below speeds of 25 Mbps download speed and 3 Mbps upload speed are now subject to a uniform reduced public comment period of 10 days and an automatic grant period of 25 days. Second, all carriers, whether dominant or non-dominant, seeking authorization to discontinue data services below speeds of 25 Mbps download speed and 3 Mbps upload speed that have previously been grandfathered for a period of at least 180 days are subject to a uniform reduced public comment period of 10 days and an automatic grant period of 31 days, provided they submit a statement as part of their discontinuance application that they have received Commission authority to grandfather the services at issue at least 180 days prior to the filing of the discontinuance application. This statement must reference the file number of the prior Commission authorization to grandfather the services the carrier now seeks to permanently discontinue. Third, carriers are no longer required to file an application to discontinue, reduce, or impair any service for which it has had no customers and no request for service for at least a 30-day period immediately preceding the discontinuance. Fourth, all carriers, whether dominant or non-dominant, that seek approval to discontinue legacy voice service can obtain further streamlined processing with a public comment period of 15 days and an automatic grant period of 31 days, provided (1) they offer a stand-alone interconnected VoIP service throughout the service area, and (2) at least one alternative stand-alone, facilities-based voice service is available from an unaffiliated provider throughout the affected service area (the “alternative options test”). Finally, all carriers, whether dominant or non-dominant, that seek approval to grandfather legacy voice service are now subject to a uniform reduced public comment period of 10 days and an automatic grant period of 25 days.

Abstract

Each operator which seeks to obtain, or is in possession of, an air carrier operating certificate is mandated to comply with the requirements of FAR Part 121 in order to maintain (recordkeeping, reporting) data which is used to determine if the air carrier is operating in accordance with minimum safety standards. Original certification is completed in accordance with part 119. Continuing certification is completed in accordance with part 121. The information reviewed as a part of this collection (manuals and guidance documents) is used to determine air operators’ compliance with the minimum safety standards and the applicants’ eligibility for air operations certification. The FAA will use the information it reviews to ensure compliance and adherence to regulations and, if necessary, take enforcement action on violators of the regulations. The information collected will not be disseminated to the public or used to support publicly disseminated information.

Key Information

Abstract

The OGE Form 201 collects certain information from, and provides certain information to, persons who seek access to public financial disclosure reports and to other covered records. The form, with the proposed modifications, reflects the requirements of the Ethics in Government Act, including the Representative Louise McIntosh Slaughter Stop Trading on Congressional Knowledge Act (STOCK Act), as amended in 2013. These requirements relate to information collected about the identity of the requester, as well as any other person on whose behalf a record is sought, and requires that the requester acknowledge his or her awareness of the legal uses of the information contained in public financial disclosure reports and potential civil penalties for persons who obtain or use a public financial disclosure report for a prohibited purpose. The form also alerts requestors that intentional falsification of the information required by the form may result in prosecution under 18 U.S.C. section 1001. Requestors are mostly members of the public, particularly representatives of the news media, public interest groups, and private citizens, with some Federal agencies and employees.

Abstract

In order to process applications for the Air Force Reserve Officers' Training Corps (AFROTC) Program and Scholarship Eligibility, information must be collected from applicants and verified for eligibility prior to acceptance in the program. AFROTC application data is collected from potential ROTC cadets (at the high school level) using an online application accessed via a web capable device. Once the applicant submits all required information and is deemed eligible by ROTC Headquarters personnel, a notification is sent to the applicant via email regarding eligibility for the AFROTC Program and, if applicable, the High School Scholarship. Once the scholarship applicant becomes eligible, the applicant is then contacted by a local AFROTC Detachment to set up an interview. When the interview is completed, then the applicant is placed on the first available scholarship board. The applicant is updated via email as to the date the board is to be held and when they will receive the results.

Abstract

TSA developed the Certified Cargo Screening Program (CCSP) as a regulatory program to support the 2007 statutory mandate to require air carriers to screen 100 percent of cargo transported on passenger aircraft. To mitigate the burden of compliance with the statutory mandate, the CCSSSP allows shippers, indirect air carriers (IACs), and other entities to voluntarily choose to participate in a program in which TSA certifies qualified entities to screen air cargo (including off-airport) before it is tendered to air carriers for transport on passenger aircraft. The collections of information under the CCSSSP (see OMB number 1652-0053) are incorporated into this Information Collection Requirement (ICR). TSA is revising the collection in response to changing conditions in the air cargo industry. Consistent with the need to expand screening capabilities to meet the International Civil Aviation Organization (ICAO) standards that take effect in 2021, Congress subsequently amended TSA’s authorities to require development of a program for TSA to certify third-party canine-cargo (3PK9-C) Teams to screen air cargo. TSA incorporated this capability under the framework of the CCSSSP. As a result, canine team providers can choose to be regulated as CCSFs under 49 CFR part 1549. As holders of a Certified Cargo Screening Program-Canine (CCSP-K9), they can contract with air carriers and other CCSFs to screen air cargo with Certified 3PK9-C Teams.

Abstract

Section 1042 of the Dodd–Frank Wall Street Reform and Consumer Protection Act, 12 U.S.C. § 5552 (“Act”), gave authority to certain State and US territorial officials to enforce the Act and regulations prescribed thereunder. Section 1042 also requires that the Bureau of Consumer Financial Protection (“Bureau”) issue a rule establishing how states are to provide notice to the Bureau before taking action to enforce the Act (or, in emergency situations, immediately after taking such an action). In accordance with the requirements of the Act, the Bureau issued a final rule (12 CFR § 1082.1) establishing that notice should be provided at least 10 days before the filing of an action, with certain exceptions, and setting forth a limited set of information which is to be provided with the notice.

Regulation SP implements the requirements of Title V of the Gramm-Leach-Bliley Act (“GLBA”), which includes the requirement that at the time of establishing a customer relationship with a consumer and not less than annually during the continuation of such relationship, a financial institution shall provide a clear and conspicuous disclosure to such consumer of such financial institution’s policies and practices with respect to disclosing nonpublic personal information to affiliates and nonaffiliated third parties (“privacy notice”). Title V of the GLBA also provides that, unless an exception applies, a financial institution may not disclose nonpublic personal information of a consumer to a nonaffiliated third party unless the financial institution clearly and conspicuously discloses to the consumer that such information may be disclosed to such third party; the consumer is given the opportunity, before the time that such information is initially disclosed, to direct that such information not be disclosed to such third party; and the consumer is given an explanation of how the consumer can exercise that nondisclosure option (“opt out notice”).

Abstract

DGMH is responsible for monitoring the effectiveness of the Technical Instructions in diagnosing applicants with tuberculosis. To assist with this monitoring, panel physicians must submit Tuberculosis Indicator reports to CDC/DGMH annually. The reports are to be submitted electronically via REDCap, and submission instructions are e-mailed to panel physicians at the beginning of each calendar year. More than 760 panel physicians perform overseas pre-departure medical examinations at 336 panel sites. Screening for tuberculosis is an important part of the medical exam in order to diagnose active TB disease prior to arrival in the U.S. The goal of the data collection is to determine TB rates in immigrant and refugee populations migrating to the US. Data are used to improve the quality of TB screening overseas and assess the impact of CDC's TB Technical Instructions. This Revision has minor adjustments.

Improving Customer Experience (OMB Circular A-11, Section 280 Implementation)

Abstract

On September 11, 1993, President Clinton issued Executive Order 12862, “Setting Customer Service Standards” which clearly define his vision that the Federal agencies will put the people first. Executive Order 12862 directs Federal agencies to provide service to the public that matches or exceeds the best service available in the private sector. Section 1(b) of Executive Order 12862 requires government agencies to “survey customers to determine the kind and quality of services they want and their level of satisfaction with existing services” and Section 1(e) requires agencies “survey front-line employees on barriers to, and ideas for, matching the best in business.” On March 30, 2016, President Obama established the Core Federal Services Council, which again emphasized the need to deliver world-class customer service to the American people. The Council, composed of the major high-volume, high-impact Federal programs that provide transactional services directly to the public, were encouraged “to improve the customer experience by using public and private sector management best practices, such as conducting self-assessments and journey mapping, collecting transactional feedback data, and sharing such data with frontline and other staff.” In March 2018, the Administration of President Trump launched the President’s Management Agenda (PMA) and established new Cross-Agency Priority (CAP) Goals. Excellent service was established as a core component of the mission, service, stewardship model that frames the entire PMA, embedding a customer-focused approach in all of the PMA’s initiatives. This model was also included in the 2018 update of the Federal Performance Framework in Circular A-11, ensuring ‘excellent service’ as a focus in future agency strategic planning efforts. The PMA included a CAP Goal on Improving Customer Experience with Federal Services, with a primary strategy to drive improvements within 25 of the nation’s highest impact programs. This effort is supported by an interagency team and guidance in Circular A-11 requiring the collection of customer feedback data and increasing the use of industry best practices to conduct customer research. In December 2021, President Biden issued Executive Order 14058: Transforming Federal Customer Experience and Service Delivery to Rebuild Trust in Government, which defines actions all agencies should take to improve customer experience through performance plans, digital modernization, and information collections. The President’s Management Agenda also names as a key priority delivering excellent, equitable and secure Federal services and customer experience. These Presidential actions and requirements establish an ongoing process of collecting customer insights and using them to improve services. This new request will enable the Institute of Museum and Library Services (hereafter “the Agency”) to act in accordance with OMB Circular A-11 Section 280 to ultimately transform the experience of its customers to improve both efficiency and mission delivery, and increase accountability by communicating about these efforts with the public.

RD23-5 The Reliability Standard PRC-0023-6 revisions will result in a change in how relay settings will be assessed under Requirement R1 of out-of-step blocking elements but will not result in reporting or recordkeeping requirements or burden. As of February 2024, there are 324 transmission owners, 1,173 generator owners, 371 distribution providers and 62 planning coordinators registered with NERC. These registered entities will have to comply requirements in the proposed Reliability Standard PRC-023-6.

Part 54 – Rate-of-Return Carrier Universal Service Reporting Requirements; Waiver of Local Exchange Carrier Study Area Boundary Changes

Abstract

In order to allow determination of the study areas that are entitled to an expense adjustment, and the wire centers that are entitled to high-cost universal service support, incumbent and competitive telecommunications carriers must provide certain data to the National Exchange Carrier Association or the Universal Service Administrative Company annually. Local telecommunications carriers who want to participate in the federal universal service support program must make certain informational showings to demonstrate eligibility. The data collected on FCC Forms 507, 508, and 509, will include line counts, forecasted cost and revenues, and actual cost and revenue data associated with consumer broadband-only loops necessary for the calculation of universal service high-cost support. In addition, the Commission now requires local exchange carriers seeking a change in study area boundaries to submit a petition for waiver of these boundary changes notwithstanding any prior exemptions from such waiver requests.

[NCCDPHP] Social and Economic Barriers to Receiving Optimal Services Along the Cancer Care Continuum

Key Information

Abstract

This research study will help CDC examine the social and economic barriers that individuals who are newly diagnosed with colorectal, breast, and cervical cancer face at each stage of the cancer care continuum, from screening through survivorship. We are also interested in learning about caregivers’ experiences including the barriers they faced as well as the barriers they perceived their care recipients faced. This project will improve our understanding of the cancer care experience, and how experiences may vary across cancer care continuum and for different populations.The results will produce data-driven evidence that inform efforts aimed at increasing access to cancer care services, reducing the burden of cancers and closing the disparities gap.
