What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

The Natural Gas Distribution Infrastructure Safety and Modernization Grant Program

Key Information

Abstract

This information collection covers the collection of applicant data from municipality and community-owned utilities that are interested in applying to receive funds from the “Natural Gas Distribution Infrastructure Safety and Modernization Grant Program.” Solicitation for grants under the Natural Gas Distribution Infrastructure Safety and Modernization Grant Program is voluntary. No eligible entity is required to apply. To be eligible, however, municipality and community-owned utilities must meet all the requirements set forth in the law. Therefore, DOT must collect certain information from applicants to determine eligibility and evaluate applications. DOT must also verify the accuracy of grant requests from approved applicants, in accordance with Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, and other laws and regulations governing Federal financial assistance programs, including (but not limited to) the Anti Deficiency Act, the Federal Funding Accountability and Transparency Act (FFATA), the Payment Integrity Information Act of 2019, and 2 CFR part 200, among others. This information collection also covers the collection of data from grant recipients. PHMSA expects to receive approximately 100 applications from potential grantees. PHMSA estimates that it will take the 100 applicants approximately 65 hours to compile and submit the forms required to complete the application process for an annual burden of 6,500 hours. PHMSA estimates that 100 grant recipients will spend 5 hours, annually, submitting post-award reports for an annual burden of 500 hours. Therefore, PHMSA estimates that there will be a total of 200 responses (100 applicants + 100 grant recipients) for an aggregate total annual burden for the information collection of 7,000 hours (6,500 hours for applications + 500 hours for post-award reports).

Abstract

This information collection mandate reported by those commercial space launch services providers seeking a license or permit, enables FAA/AST to determine the maximum probable loss (MPL) resulting from licensed or permitted launch activities. The information is collected once based on vehicle configuration, launch site, and flight trajectory that are introduced in the license or permit application. The collected information will be used to determine if licensees have complied with financial responsibility requirements for MPL analysis as set forth in FAA regulations (§ 440.7(c)). The FAA is responsible for determining MPL required to covered claims by a third party for bodily injury or property damage, and the United States, its agencies, and its contractors and subcontractors for covered property damage or loss, resulting from a Commercial space transportation permitted or licensed activity. The MPL determination forms the basis for financial responsibility requirements issued in a license or permit order. The licensee or permittee provides the MPL results to an insurance broker to purchase insurance for the license or permit mission. The following is a summary of the key information required to conduct an MPL: 1. Mission description. • Launch trajectory; • Orbital inclination; and • Orbit altitudes (apogee and perigee). 2. Flight sequence. 3. Staging events and the time for each event. 4. Impact locations. 5. Abstract

Section 731 of Title VII of the Dodd-Frank Wall Street Reform and Consumer Protection Act (Dodd-Frank Act, Pub L. No. 111-203, 124 Stat. 1376 (2010)) amended the Commodity Exchange Act (CEA) by adding sections 4s(f), 4s(g), and 4s(i), among others. Pursuant to this authority, the Commission promulgated regulations 23.500-23.505, which obligate swap dealers (SDs) and major swap participants (MSPs) to develop and retain written swap trading relationship documentation. The final regulations also establish requirements for SDs and MSPs regarding swap confirmation, portfolio reconciliation, and portfolio compression. Confirmation, portfolio reconciliation, and portfolio compression are important, post-trade processing mechanisms for reducing risk and improving operational efficiency. *Note: The collections of information under OMB control numbers 3038-0068, 3038-0083, and 3038-0088 are now consolidated under OMB control number 3038-0088, and OMB control numbers 3038-0068 and 3038-0083 have been withdrawn as of July 5, 2022. Concurrently with this change, the Commission has changed the name associated with OMB control number 3038-0088 to “Swap Documentation.” While the collection has been renewed until June 30, 2025, this amended supporting statement is now submitted to correct the aggregate burden hours for the consolidated collection, and to officially affirm the discontinuance of OMB control numbers 3038-0068 and 3038-0083.

Senior Loan Officer Opinion Survey on Bank Lending Practices

Key Information

Abstract

A senior loan officer at each respondent bank completes this voluntary survey through an electronic submission, up to six times a year. Consistent with the Senior Financial Officer Survey (FR 2023; OMB No. 7100-0223), senior staff at the Reserve Banks with knowledge of bank lending practices serve as the main contacts for Senior Loan Officer Opinion Survey on Bank Lending Practices (SLOOS) respondents in their district and help administer the survey. The current reporting panel consists of up to 80 large domestically chartered commercial banks and up to 24 large U.S. branches and agencies of foreign banks. The purpose of the survey is to provide qualitative and limited quantitative information on credit availability and demand, as well as on evolving developments and lending practices in the U.S. loan markets. A portion of each survey typically covers special topics of timely interest. Although the Board has the authority to conduct the survey up to six times a year, the survey has typically been conducted only four times a year since 1992. Consistent with the FR 2023, other types of respondents, such as other depository institutions, bank holding companies, or other financial entities, may be surveyed, if appropriate. The respondents’ answers provide information that is critical to the Federal Reserve System’s monitoring of bank lending practices and credit markets. The Federal Reserve relies on the regular opportunity to solicit information from banks within the framework of the survey. Aggregated survey results from 1997 to present are available to the public on the Board’s website.

This is a request for a rule-related (0648-BN23) revision and extension of the OMB Control No. 0648-0605. This revision request makes the following changes: • Removal of weekly annual catch entitlement (ACE) reporting requirements for sectors (50 CFR 648.87(b)(1)(v)(B)); • Removal of the option to increase the frequency of ACE reporting requirements for sectors when the balance of remaining ACE is low, as specified in the sector operations plan and approved by NMFS (50 CFR 648.87(b)(1)(v)(B)); • Removal of the requirement for sectors to include in their operations plan a list of all Federal and state permits held by persons participating in the sector, including an indication for each permit whether it is enrolled and will actively fish in a sector, or will be subject to the provisions of the common pool (50 CFR 648.87(b)(2)(ii)). These collections are necessary to carry out the requirements of the groundfish fishery as specified by the Northeast (NE) Multispecies Fishery Management Plan (FMP). Abstract

FERC-545 is required to implement sections 4, 5, and 16 of the Natural Gas Act (NGA), (15 USC 717c, 717d, and 717o). NGA Sections 4, 5, and 16 authorize the Commission to inquire into rate structures and methodologies and to set rates at a just and reasonable level. Specifically, a natural gas company must obtain Commission authorization for all rates and charges made, demanded, or received in connection with the transportation or sale of natural gas in interstate commerce. In the NOPR, the Commission proposes to amend its regulations at 18 CFR 284.12 to incorporate by reference, with certain enumerated exceptions, Version 4.0 of Standards for Business Practices of Interstate Natural Gas Pipelines adopted by the Wholesale Gas Quadrant of the North American Energy Standards Board (NAESB). These amendments, if implemented, would affect FERC-545 by requiring filers to establish compliant mechanisms to share and post information which would be included in their rates on file with the Commission.

Abstract

Advisory Circular (AC) 120-119, Voluntary Safety Management System for Other Regulated Entities Transporting Dangerous Goods by Air, provides information on how entities subject to Title 49 of the Code of Federal Regulations (CFR) Parts 171-180 may voluntarily implement a Safety Management System (SMS) as described in 14 CFR Part 5, Safety Management Systems. These entities perform functions such as handling or shipping dangerous goods by air and are referred to as Other Regulated Entities (ORE). As outlined in AC 120-119, large OREs can use the SMS principles in 14 CFR Part 5 as a basis to develop a voluntary SMS program, including an SMS Implementation Plan, which is submitted to the FAA’s Office of Hazardous Materials Safety (AXH) for review and acceptance. Then, the ORE implements its SMS program and maintains records to document processes, identify hazards, and document non-compliance. This collection covers the burden of developing the implementation plan and continued monitoring of the voluntary SMS program. Changes in this renewal include: • Adjustment (added an IC) to better distinguish between initial submission and continuing recordkeeping burden; • Adjustment in respondent estimates (decrease) based on current data; and • Adjustment in annual burden estimates for initial submission (decrease) based on subject matter expertise.

Medicare Health Outcomes Survey Field Test (CMS-10861)

Abstract

The Centers for Medicare & Medicaid Services (CMS) has fielded the Medicare Health Outcomes Survey (HOS) annually since 1998. The HOS is a national survey of Medicare managed care plan enrollees that provides data that permit the calculation of both longitudinal and cross-sectional measures of plan performance that are publicly reported to assist Medicare beneficiaries in making enrollment decisions. This request is to conduct a field test to evaluate potential new survey items, alternatives to existing survey content, and the effects of a web-based survey administration mode on response patterns and measure scores. CMS’s goal is a refined and shorter HOS survey instrument with new and methodologically simpler cross-sectional and longitudinal measures, expanded measurement of physical functioning and mental health, and the addition of survey items that support CMS’s efforts to measure and address health equity.

Abstract

Title 14 CFR Part 157 mandates that all “persons proposing to construct, alter, activate, or deactivate a civil or joint-use (civil/military) airport or to alter the status or use of such an airport” must notify the FAA before any construction, alteration, or change to the status or use of an airport. Airports submit this information using FAA Form 7480-1, Notice for Construction, Alteration and Deactivation of Airports, or its online equivalent. The collection is reporting and occurs on occasion as needed. The collection requires proponents to report and certify the purpose of the notification; the name, location, use, and type of landing area affected; landing area data; and operation data, including number of based aircraft and average number of landings. The FAA uses the information collected when: • Determining the effect the proposed action will have on existing airports and on the safe and efficient use of airspace by aircraft. • Determining the effects the proposed action will have on existing or contemplated traffic patterns of neighboring airports. • Determining the effects the proposed action will have on the existing airspace structure and projected programs of the FAA. • Determining the effects that existing or proposed manmade objects (on file with the FAA) and natural objects within the affected area will have on the airport proposal. Proponents are also required to notify the FAA by letter or by FAA Form 5010-5 within 15 days after completion of the project. The FAA requires this notification because many proposals are never completed for reasons pertinent only to the sponsor. The confirmation of the completion of the project is needed in order to identify the cancelled proposals, so as to release any airspace that may have been reserved and to update aeronautical charts and flight information publications. The collection/burden for Form 5010-5 falls under a different collection (OMB Control Number 2120-0015). It is anticipated that the information collected will be disseminated to the public or used to support publicly disseminated information in aeronautical charts and flight information publications. FAA Office of Airports will retain control over the information and safeguard it from improper access, modification, and destruction, consistent with FAA standards for privacy and electronic information. See response to Question 10 of this Supporting Statement for more information on confidentiality and privacy. The information collection is designed to yield data that meet all applicable information quality guidelines.

Abstract

The Patient Safety and Quality Improvement Act of 2005 (hereafter the Patient Safety Act), 42 U.S.C. 299b-21 to 299b-26, was enacted in response to growing concern about patient safety in the United States and the Institute of Medicine's 1999 report, To Err is Human: Building a Safer Health System. The goal of the statute is to improve patient safety by providing an incentive for health care providers to work voluntarily with experts in patient safety to reduce risks and hazards to the safety and quality of patient care. The Patient Safety Act signifies the Federal Government's commitment to fostering a culture of patient safety among health care providers; it offers a mechanism for creating an environment in which the causes of risks and hazards to patient safety can be thoroughly and honestly examined and discussed without fear of penalties and liabilities. It provides for the voluntary formation of Patient Safety Organizations (PSOs) that can collect, aggregate, and analyze confidential information reported voluntarily by health care providers. By analyzing substantial amounts of patient safety event information across multiple institutions, PSOs will be able to identify patterns of failures and propose measures to eliminate or reduce patient safety risks and hazards.
