An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public.
Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.
Federal agencies are required to submit an ICR whenever they create, renew, or modify an information collection. Each ICR includes a description of the collection,
supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.
The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after
which the collection must be renewed through a new ICR submission.
ICRs are publicly available on RegInfo.gov, and additional guidance can be found in the FAQs.

Abstract
Section 204(d) of the Act provides that if an individual dies before payment under Title II is complete, or before a Medicare premium refund is due, SSA will pay the amount due (including the amount of any check not negotiated) to people who meet specified qualifications under an order of priority. When a Social Security payment, or Medicare premium, was due to a deceased beneficiary at the time of death, and there is insufficient information in the file to identify the people entitled to the payment, or their addresses, SSA asks the surviving spouse, next of kin, or legal representative of the estate to complete Form SSA-1724, Claim for Amounts Due in the Case of a Deceased Beneficiary. SSA collects the information when a surviving child(ren), parent(s), or spouse is not already entitled to a monthly benefit on the same earnings record, or is not filing for a lump-sum death payment as a former spouse. SSA uses the information Form SSA-1724 provides to ensure proper payment of an underpayment due to a deceased beneficiary. The respondents are applicants for Title II underpayments or Medicare premium refunds owed to deceased beneficiaries. |

Abstract
This request is for revision and extension of this information collection. NOAA is proposing changes to both the youth and general advisory council applications and the changes are outlined in response to Question 1. Section 315 of the National Marine Sanctuaries Act (NMSA) (16 U.S.C. § 1445a) authorizes the Secretary of Commerce to establish one or more advisory councils to advise and make recommendations to the Secretary regarding the designation and management of national marine sanctuaries. Executive Order 13178 (December 4, 2000) similarly established a Coral Reef Ecosystem Reserve Council pursuant to the NMSA to provide advice and recommendations regarding the Northwestern Hawaiian Islands Coral Reef Ecosystem Reserve. Councils are individually chartered to meet the specific needs of a sanctuary site or of the sanctuary system; 16 councils–15 sanctuary advisory councils and one reserve advisory council—currently exist. Once a council has been chartered, and as vacancies occur for existing councils, the Office of National Marine Sanctuaries starts a process to recruit members for that council by providing notice to the public and asking interested parties to apply for the available seats. An application form has been developed to help ease the application process for the public and facilitate the review process for the sanctuary superintendent. |

18 Stat. 1852 Abstract
Section 1852(e) of the Social Security Act requires that MA organizations (MAOs) have an ongoing quality improvement program. CMS regulations at 42 CFR §422.152 outline the quality improvement program (QI Program) requirements for MA plans, which includes the development and implementation of both a Quality Improvement Project (QIP) and a Chronic Care Improvement Program (CCIP). The 2014 approved packaged reflected a process that required plans to report QIP and CCIP projects’ progress reports annually. This reinstatement package outlines the burden for the attestation process that reduces burden by requiring plans to attest annually that they have an ongoing CCIP that meets the requirements outlined at 42 CFR §422.152, rather than report annual progress reports. MA plans will only have to attest to having a CCIP once the proposed rule (CMS-4182-F; RIN 0938-AT08) that removes the QIP is final. |

Abstract
The Lacey Act (Act, 18 U.S.C. 42) prohibits the importation of any animal deemed to be and prescribed by regulation to be injurious to: • Human beings; • The interests of agriculture, horticulture, and forestry; or • Wildlife or the wildlife resources of the United States. Implementation and enforcement of the Lacey Act is the responsibility of the Department of the Interior. The Service, in concert with our diverse partners, works to conserve, restore, and maintain the nation's fishery resources and aquatic ecosystems for the benefit of the American people, to include managing and controlling four species of invasive carp—bighead, black, grass, and silver—native to Asia. Under the authority of the Act, the Service listed bighead, black, and silver carp species as injurious wildlife to protect humans, native wildlife, and wildlife resources from the purposeful or accidental introduction of invasive carp into the nation's aquatic ecosystems. The Service takes part in a broad, partner-driven approach to strategically control the movement of invasive carp. The spread of these invasive species in the nation's river systems threatens the conservation efforts conducted by our agency, our State partners, and other stakeholders, to promote self-sustaining aquatic resources and healthy aquatic ecosystems. In addition to widespread and longstanding ecological consequences, aquatic invasive species often result in significant economic losses and cost our nation's economy billions of dollars per year. To effectively carry out our responsibilities under the Act and protect the aquatic resources of the United States, the Service, in collaboration with the U.S. Geological Survey, proposes to administer applications of Carbon Dioxide-Carp by registered management partners (applicators) and to collect information regarding the usage of Carbon Dioxide-Carp, an Environmental Protection Agency (EPA) registered product #6704-95 to control invasive carp. Abstract
The Migratory Bird Treaty Act (16 U.S.C. 703–712) designates the Department of the Interior as the primary agency responsible for managing migratory bird populations frequenting the United States and setting hunting regulations that allow for the well-being of migratory bird populations. These responsibilities dictate that we gather accurate data on various characteristics of migratory bird populations. The Service’s fall survey for eastern population sandhill crane was established in 1979. It is implemented by state and Federal agencies and public volunteers from eight states in the Atlantic and Mississippi Flyways, as well as Ontario, Canada. Sandhill cranes are widely dispersed during the breeding and wintering seasons and are difficult to count. The optimal time to survey cranes is during the last week of October when the majority of eastern population cranes breeding in Canada migrate to traditional staging grounds in the Great Lake States (e.g. Jasper-Pulaski Fish and Wildlife Area, Medaryville, Indiana). Since the initial survey in 1979, crane numbers have increased to over 90,000 birds. |

The SAFECOM Membership Questionnaire is an internal SAFECOM document disseminated only to active SAFECOM Members. SAFECOM uses the Questionnaire to identify membership gaps, obtain updated information on SAFECOM’s membership body (e.g., public safety communications experience, accolades, acquired skills/certifications, etc.), update SAFECOM marketing materials, and to assist SAFECOM when responding General Accounting Office (GAO) inquiries. The Questionnaire will encompass eight interdependent sections of questions. The SAFECOM Internal Membership section requests each Member to provide their name, state of residence, and the number of hours per month he/she contributes SAFECOM led initiatives (e.g., conference calls and deliverable development). SAFECOM consists of public safety association representatives and at-large members. The Association Representative Information section pertains to public safety associations represented in SAFECOM. Association Representatives serving in SAFECOM are asked to provide the name of their Association, approximate Association size, Association contact, and addition Association point-of-contact (POC) information. At-large members are instructed to skip to the next section. Public Safety Service section will focus on questions related to each Member’s public safety and first responder career. The Speaker Request Form will be the first point of contact between CISA and the public to initiate CISA speaking engagements. The form will be available on www.cisa.gov and any member of the public can submit a request for a CISA employee to speak at an event. The form will be used by CISA to track and manage external speaking engagements. The information will be used to schedule and determine the most appropriate CISA speaker based on date, time, location, presentation format, and topic. The form collects information about the event, discussion topics, presentation format, media participation, disclosures, special event guest, and event coordinator information. Information from the Speaker Request Form regarding the requested speaking engagement, the host organization, the speaking topic, agenda and additional event details will be collected. The requested information helps CISA determine whether the speaker should attend the engagement and/or how CISA should best prepare for the event. The information is used to determine if the event warrants the time and attention of the invited speaker and what messages and information can be shared on behalf of the Agency with a public or targeted audience. This request is for an extension of an existing information collection under OMB Control No. 0648-0151. NMFS does not anticipate that the extension will substantially add to the burden to individual private applicants for incidental take authorizations under the existing Control Number. The main difference we expect related to this extension is that there will be a larger number of applicants/respondents than accounted for in the existing OMB Control Number. In preparing this renewal, it was observed that some totals in the previous Supporting Statement were inaccurate. These have been corrected in this document. The Marine Mammal Protection Act of 1972 (MMPA; 16 U.S.C. 1361 et seq.) prohibits the "taking" of marine mammals unless otherwise authorized or exempted by law. The National Marine Fisheries Service (NMFS) uses the information collection to: evaluate the proposed activity's impact on marine mammals; arrive at the appropriate determinations required by the MMPA and other applicable laws prior to issuing or denying the authorization; and monitor impacts of activities for which take authorizations have been issued. |
