Change Requests

What is an ICR?

An Information Collection Request (ICR) is a federal agency's request for approval from the Office of Management and Budget (OMB) to collect information from the public. Under the Paperwork Reduction Act (PRA), agencies must justify why the information is needed and how it will be used.

When are they submitted?

Federal agencies are required to submit an ICR whenever they create, renew, modify an information collection. Each ICR includes a description of the collection, supporting materials and documentation (such as forms, surveys, or scripts), and proof that the agency has met the requirements of the PRA.

The ICR is submitted to the The Office of Information and Regulatory Affairs (OIRA) within OMB for review and approval. OIRA grants approval for a maximum of three years, after which the collection must be renewed through a new ICR submission.

National Healthy Start Evaluation and Quality Assurance

Key Information

Abstract

The goal of this ongoing monitoring and evaluation is to assess Healthy Start (HS) progress towards meeting its performance goals. Results from monitoring and evaluation efforts will provide actionable evidence to support the improvement, sustainability, replication, and dissemination of the program. The information collected will be used to obtain consistent information across all 100 grantees about HS, its participants, and its outcomes. Respondents include pregnant women and women of reproductive age that were served by HS and HS staff. This revision seeks to reduce the burden of data collection on grantees and program participants, as well as to focus the evaluation more clearly on progress towards meeting performance goals regarding program impact. In the process, this will ensure that collected data is meaningful for monitoring and evaluation, and that previously separate data systems can be streamlined, thus improving efficiency.

Maternal, Infant, and Early Childhood Home Visiting Program Performance Measurement Information System

Key Information

Abstract

This request is for continued approval of the Maternal, Infant, and Early Childhood Home Visiting (MIECHV) Program Performance Measurement Information System. HRSA uses performance information to demonstrate program accountability and continuously monitor and provide oversight to MIECHV Program awardees. The information is also used to provide quality improvement guidance and technical assistance to awardees and help inform the development of early childhood systems at the national, state, and local level. HRSA is seeking to continue to collect information on demographic, service utilization, and select clinical indicators for participants enrolled in home visiting services and a set of standardized performance and outcome indicators that correspond with the statutorily identified benchmark areas. This information will be used to demonstrate awardees’ compliance with legislative and programmatic requirements. It will also be used to monitor and provide continued oversight for awardee performance and to target technical assistance resources to awardees.

[NCCDPHP] Message Testing for Tobacco Communication Activities

Key Information

Abstract

In 2012, CDC/OSH obtained OMB approval of a generic clearance to support the development of tobacco-related health messages (Message Testing for Tobacco Communication Activities (MTTCA), OMB No. 0920-0910). The MTTCA clearance has been used to obtain OMB approval for a variety of message testing activities. CDC is requesting OMB approval to extend the generic MTTCA clearance for three years. CDC will continue to use the MTTCA clearance to develop and test messages and materials for current and future phases of the ACA-funded media campaign, as well as OSH's ongoing programmatic initiatives including, but not limited to, the Media Campaign Resource Center, reports from the Office of the Surgeon General, and other communication efforts and materials. The MTTCA generic clearance may also be used to facilitate the development of tobacco-related health communications of interest for CDC collaborative efforts with other federal partners including, but not limited to, the Food and Drug Administration, the Substance Abuse and Mental Health Services Administration, the National Institutes of Health, and the National Cancer Institute. The MTTCA clearance is sufficient to test tobacco related messages developed by CDC. No modification is requested for information collection activities or methodology. Non-Subsantive Change Request is submitted to modify questions in previously approved GenIC for compliance with recently issued EOs. There is no change to burden hours.

The results of the survey will be used to identify objectives and priorities for intervention research to improve the safety of the workplace, reduce injury, and improve the well-being of applied behavior analysis workers. Culture of Continuous Learning Project: Case Study

Abstract

On March 17, 2023, we received OMB approval to conduct a descriptive case study for the Culture of Continuous Learning Project: Case Study (OMB #0970-0605). The purpose of this project is to document the factors that contribute to the feasibility of implementing the Breakthrough Series Collaborative (BSC) quality improvement methodology in Head Start and child care centers. The OMB-approved information collection includes: (1) implementation materials (e.g., worksheets); (2) key informant interviews; (3) focus groups with key staff, teachers, and parents; and (4) surveys with administrators, teachers, staff, and parents. The authorization to require a Lead Agency in the State or Territory administering a Child Care and Development Fund (CCDF) program to submit a plan using the Preprint form is contained in the Child Care and Development Block Grant Act of 1990 (CCDBG Act), 42 U.S.C 9858 as amended by the Child Care and Development Block Grant Act of 2014, Public Law 113-186. The National Healthcare Safety Network (NHSN) is a system designed to accumulate, exchange, and integrate relevant information and resources among private and public stakeholders to support local and national efforts to protect patients and promote healthcare safety. Specifically, the data is used to determine the magnitude of various healthcare-associated adverse events and trends in the rates of these events among patients and healthcare workers with similar risks. The data will be used to detect changes in the epidemiology of adverse events resulting from new and current medical therapies and changing risks. This Change Request is submitted to adhere to recent EOs and includes another minor change.Emergency Justfication:Emergency Processing is requested to add three (3) additional pathogens of concern to the NHSN (OMB Control No. 0920-0666) data collection: Influenza A (H5), Marburg, and Oropouche. This requested update in disease surveillance will illustrate which patient populations are being affected by these pathogens and the potential need for additional healthcare infection prevention and control. Since this form is collecting data on hospitalized patients, the update will also help inform on the severity of illness a high consequence pathogen is causing as well as what region(s) of the country are affected. Data collection is vital to ensure that local and state authorities are supported to prepare for and respond to emerging outbreaks.

[NIOSH] Reducing Fatigue Among Taxi/Rideshare Drivers

Key Information

Abstract

The goal of this study is to evaluate an online, interactive training on fatigue management tailored to taxi drivers and other for-hire rideshare drivers. The proposed study is a longitudinal, randomized design comparing two interventions and a control over 5 study periods. One group of 60 drivers will receive both interventions: fatigue training and daily feedback on estimated fatigue score. A second group of 60 drivers will receive only the fatigue training. A third group of 60 drivers will receive neither intervention. All participating drivers will take a self-administered questionnaire, wear a wrist actigraph to measure sleep quality, complete a sleep and activities diary, and take a sleep health and driving knowledge assessment. Drivers taking the training will be asked to complete an evaluation. Findings will be used to improve the training program for potential future dissemination. This change request updates documents to follow executive orders.

Fatality Analysis Reporting System (FARS)

Key Information

Abstract

The FARS is a voluntary information collection of fatal motor vehicle traffic crashes. The FARS is in its forty-sixth year of operation and is a census of all defined crashes involving fatalities in the country. The FARS collects data from all 50 States, the District of Columbia, and Puerto Rico under cooperative agreements. State employees extract and transcribe information from existing State files including police crash reports as well as driver license, vehicle registration, highway department, and vital statistics files. NHTSA aggregates the data for research and analysis in support of motor vehicle regulations and highway safety programs. This supports NHTSA’s mission to save lives, prevent injuries, and reduce economic losses resulting from motor vehicle crashes by providing the agency vital information about fatal crashes. The aggregated data comprises a national database that is NHTSA’s and many States’ principal means of tracking trends in fatalities and quantifying problems or potential problems in highway safety. The FARS data are used extensively by all the NHTSA program and research offices, other DOT modes, States, and local jurisdictions. The highway research community uses the FARS data for trend analysis, problem identification, and program evaluation. Congress uses the FARS data for making decisions concerning safety programs. The FARS data are also available upon request to anyone interested in highway safety. The annual burden has been adjusted from 106,244 to 107,209 hours (an increase of 965 hours) costs have decreased from $100,000 to $0 (a decrease of $100,000). The adjustment in burden hours is due to the increase in the complexity of coding the FARS cases and an increase in the number of fatal crashes across most States. The increase also accounts for the time to process the non-traffic fatalities for NTS. Furthermore, while time for manually inputting data has decreased with States implementing systems to electronically transfer police report data that prepopulate NHTSA’s data systems, including FARS, the overall burden increased because, over the past two years, there has been an increase in staff turnover at the State level, adding an increase in administrative hours, training, and coding assistance to continue operations. The decrease in costs is a result of removing labor costs associated with labor hours that were incorrectly included in our last ICR.

Abstract

Under the Youth Conservation Corps Act of August 13, 1970, as amended (U.S. 1701–1706), the U.S. Department of Interior and the U.S. Department of Agriculture-U.S. Forest Service provide seasonal employment for eligible youth 15 through 18 years old. The Youth Conservation Corps stresses three important objectives: 1. Accomplish needed conservation work on public lands; 2. Provide gainful employment for 15 to 18 year-old males and females from all social, economic, ethnic, and racial backgrounds; and 3. Foster, on the part of the 15 through 18 year-old youth, an understanding and appreciation of the Nation’s natural resources and heritage. Youths seeking training and employment with the Youth Conservation Corps must complete the following new common forms included in this emergency clearance request: DI-4014, “Youth Conservation Corps Application” and DI-4015, “Youth Conservation Corps Medical History.” The applicants’ parents or guardians must sign both forms. The application and medical history forms are evaluated by participating agencies to determine the eligibility of each youth for employment with the Youth Conservation Corps. Potential and actual agencies that may use the common forms included in this collection include: • U.S. Fish and Wildlife Service (Interior); • National Park Service (Interior); and, • Other Federal Departments and Agencies such as the U.S. Forest Service (U.S. Department of Agriculture).

Ownership Report for Noncommercial Educational Broadcast Stations, FCC Form 323-E; Section 73.3615, Ownership Reports

Key Information

Abstract

On January 8, 2016, the Commission adopted a Report and Order, Second Report and Order, and Order on Reconsideration, FCC 16-1 (323/CORES Order and Reconsideration Order). The 323/CORES Order and Reconsideration Order adopted changes to the ownership reporting requirements that are intended to further improve the ownership data collection. The 323/CORES Order and Reconsideration Order adopts a Restricted Use FRN within CORES that individuals may use solely for the purpose of broadcast ownership report filings. In light of the Commission’s adoption of the Restricted Use FRN, the 323/CORES Order and Reconsideration Order eliminates the availability of the interim Special Use FRN for broadcast station ownership reports, except in very limited circumstances. The 323/CORES Order and Reconsideration Order prescribes revisions to Form 323-E that conform the reporting requirements for noncommercial educational (NCE) broadcast stations more closely to those for commercial stations, including revisions to collect race, gender, and ethnicity information for attributable interest holders; require that CORES FRNs or Restricted Use FRNs be used; and conform the biennial filing deadline for NCE broadcast ownership reports with the biennial filing deadline for commercial station ownership reports. The 323/CORES Order and Reconsideration Order also makes a number of other improvements to the forms that are intended to reduce the filing burdens on broadcasters, streamline the filing process, and increase the quality and usability of the data submitted to the Commission. These changes include extending the biennial filing deadline, reducing the number of filings required, improving the reporting of other broadcast interests, and additional improvements. FCC Form 395-A, the “Multi-Channel Video Programming Distributor Annual Employment Report,” is a data collection device used by the Commission to assess industry employment trends and provide reports to Congress. By the form, multi-channel video programming distributors (“MVPDs”) identify employees by gender and race/ethnicity in sixteen specified major job categories in the form approved by the Office of Management and Budget (“OMB”) in 2020 and re-submitted herein for approval in 2023. Abstract

The Volunteer Income Tax Assistance (VITA) program developed the Forms 13614-C and 13614-NR that contain a standardized list of required intake questions to guide volunteers in asking taxpayers basic questions about themselves. The forms are effective tools to ensure VITA volunteers consistently collect personal information from each taxpayer to assure the tax returns are prepared accurately and avoid erroneous returns. These forms are critical to continued improvements in the accuracy of volunteer-prepared returns for taxpayers utilizing the VITA program. Form 13614-C is used to assist VITA volunteers in preparing individual tax returns for citizens or residents of the United States. The IRS also provides multiple translations of Form 13614-C. Form 13614-NR is used to assist VITA volunteers in preparing individual tax returns for nonresident aliens.

Abstract

Authorized by the NPS Organic Act, 54 U.S.C. 100101 et seq., and Public Health Service Act, 42 U.S. Code Chapter 6A, the NPS Office of Public Health (OPH) is called upon to conduct disease surveillance, respond to urgent outbreaks, and prevent illnesses within or associated with National Parks. National Parks are federally managed lands where state and local health departments may not have jurisdiction, therefore the public health response rests with the NPS OPH. The information collected will be used to determine the agents, sources, modes of transmission, or risk factors so that effective prevention and control measures can be implemented.

Abstract

The National Park Service (NPS) is authorized by the System Unit Resource Protection Act (54 U.S.C. 100721) to collect information that can be used to determine the economic value associated with the preservation (avoided loss) of individual members of a wildlife species population. The NPS Environmental Quality Division is requesting approval to conduct a survey to provide estimates of the full value of protecting individual animals from intentional or accidental loss. These value estimates are not currently available to the NPS and are necessary for park management decisions.
