Annual Report of Class I and Class II Motor Carriers of Property (OMB 2139-0004)

Abstract

The purpose of this request is to renew the currently OMB approved ICR. Form M is required to collect financial, operating, equipment, and employment data from individual motor carriers of property and household goods. All for-hire (common and contract) motor carriers of property with gross annual operating revenue of $3 million or more must file the annual report Form M. Form MP-1 is required to collect financial, operating, equipment, and employment data from individual motor carriers of passengers. All Class I for-hire motor carriers of passengers with gross annual operating revenues of $5 million or more are required to file the annual report Form MP-1. FMCSA has created electronic forms that may be prepared, signed electronically, and submitted to FMCSA via https://ask.fmcsa.dot.gov/app/ask/. However, forms can still be mailed to FMCSA. The data collected is available to users in its original form. The data are not used by the U.S. Department of Transportation, and, based on a comment to a proposed rule finalized on December 17, 2013 (78 FR 76241), the data are no longer used by trucking associations. The agency is also considering how it might remove some of this reporting requirement, consistent with statue, through a separate rulemaking, but the agency needs to renew the Collection of Information now while that process plays out. Insurance companies, consultants, law firms, academia, trade publications and others may use the data to assess industry growth and its impact on the economy, to identify industry changes that may affect national transportation, and to monitor company financial stability. In the past, the Bureau of Economic Analysis (BEA) used the data to inform the national annual input-output and Gross Domestic Product (GDP) estimates, to prepare estimates of industry output, and to provide details on inputs to supplement the information on motor carriers of passengers collected by the U.S. Census Bureau. FMCSA is not aware of anyone currently using this information. FMCSA does not use the information collected through either Form M or Form MP-1, nor does it enforce the filing requirements found in part 369. The Agency was not provided the necessary resources to administer the data collection and the financial reports are of little if any value in fulfilling the Agency’s safety mission. FMCSA estimates that the annual burden hour for this information collection has decreased by approximately 78 hours due to decreased Form M and Form MP-1 submissions received by the Agency between 2018 and 2020.

Licensing Applications for Motor Carrier Operating Authority

Key Information

Abstract

FMCSA registers for-hire motor carriers of regulated commodities and of passengers, surface freight forwarders, property brokers, and certain Mexico-domiciled motor carriers, as mandatory reporting. These motor carriers may conduct transportation services in the United States only if they are registered with FMCSA. FMCSA is still accepting forms OP-1, OP-1(P), OP-1(FF), and OP-1(NNA) for existing registrants wishing to apply for additional authorities. FMCSA requires Form OP-1(MX) for Mexico-domiciled carriers that wish to operate beyond the U.S. municipalities on the U.S.-Mexico border and their commercial zones. Information collected through these forms, as an ICR extension, aids FMCSA in determining the type of operation a company may run, the cargo it may carry, and the resulting level of insurance coverage the applicant will be required to obtain and maintain to continue its operating authority. The OP-1 series requests information on the applicant’s familiarity with relevant safety requirements, the applicant’s willingness to comply with those requirements during its operations, and the applicant’s willingness to meet any specific statutory and regulatory requirements applicable to its proposed operations. FMCSA also uses information from these forms to prepare (for each applicant) a public notice of the registration application, which is published in the “FMCSA Register.” Publication of this notice initiates a 10-day period during which an interested party may file a complaint seeking denial of the registration on a limited number of statutory grounds. Effective December 12, 2015, OP-1 series forms, with the exception of OP-1(MX), can only be used to apply for additional authorities, not for initial registration with FMCSA. These forms request information to identify the applicant, the nature and scope of its proposed operations, a narrative description of the applicant’s safety policies and procedures, and information regarding the drivers and vehicles it plans to use in U.S. operations. There is no requirement for the OP-1, OP-1(P), OP-1(FF), and OP-1(NNA) forms be filed on a periodic basis. The forms are filed on an “as needed” basis. Since the rollout of URS Phase I, the sole purpose of these forms is to allow existing registrants to apply for additional authorities. A new OP-1(MX) application form is currently required only if a Mexico-domiciled carrier wishes to extend the scope of its authority or is applying for new authority.

Application for Certificate of Registration for Foreign Motor Carriers and Foreign Motor Private Carriers

Abstract

This collection of information is used by the FMCSA to register foreign motor carriers to operate across the Mexico- U.S. border into the United States in accordance with 49 U.S.C. 13902(c) and 49 CFR 368. Data contained on the Form OP-2 will provide the FMCSA with evidence to assess the foreign (Mexico-based) motor carrier’s safety program and its ability to comply with U.S. safety standards. It also provides information concerning changes in each carrier’s operating status to ensure actual compliance with safety requirements. This enables the Agency to maintain an accurate census of registered carriers and ensure safety. There is an increase in the number of respondents and in the burden hours calculated for this renewal. The annual burden hour increase of 831 is due primarily to the increase in the number of updated OP-2 forms filed from 2020 through 2022. The average number of entities which filed updated OP-2 forms in the three-year period 2020 and 2022 increased by 95 percent, compared to the number that registered from 2017 through 2019.

Biographic Information (Registration)

Abstract

The registration and fingerprinting requirements currently appear in sections 1301-1306 of Title 8 of the United States Code. With limited exceptions, aliens above the age of 14 who remain in the United States for 30 days or longer, must apply for registration and be fingerprinted before the expiration of the 30 days, unless previously registered and fingerprinted under other provisions of immigration law. 8 U.S.C. 1302. Similarly, parents and guardians must ensure that their children below the age of 14 are registered. 8 U.S.C. 1302(b). Within 30 days of reaching his or her 14th birthday, the alien child must “apply in person for registration and to be fingerprinted.” 8 U.S.C. 1302(b). Implementing regulations can be found at 8 C.F.R. part 264. USCIS uses the electronic version of Form G-325R, Biographic Information (Registration), for the purpose of registration of unregistered aliens present in the United States in accordance with 8 U.S.C. 1302.

This is a revision request for the previously approved ICR to continue collecting program monitoring and reporting information from jurisdictions (which include states, Washington, D.C., U.S. Territories, cities, and counties) currently funded under the Overdose Data to Action (CDC-RFA-CE19-1904) notice of funding opportunity (OD2A NOFO). Reporting requirements have been carefully designed to align with and support the specific goals and outcomes outlined in the OD2A NOFO. This revision request is to initiate data collection activities for two new Overdose Data to Action NOFOs, the Overdose Data to Action in States (OD2A-S) will collect program monitoring and reporting information from states and Washington, D.C. CDC analyzes outbreak data to determine trends and develop and refine recommendations for prevention and control. This Revision is a request to combine the two currently approved forms. Motor Carrier Records Change Form

Key Information

Abstract

This information collection is mandatory reporting for which the frequency is on an as needed basis. The information is collected from for-hire motor carriers, brokers and freight forwarders who are required to notify the Office of Registration and Safety Information located within DOT’s Federal Motor Carrier Safety Administration (FMCSA), when they change the name or address of their company. The name change request may be filed online through the Licensing and Insurance website, or companies may fax or mail the form requesting either name or address changes. Carriers may also, by use of this form, request reinstatement of a revoked operating authority either online or via fax. The purpose of the collection is to enable FMCSA to maintain up-to-date records which will allow the agency to recognize the entity in question in case of enforcement actions, or other procedures required to ensure the carrier is fit, willing and able to provide for-hire transportation services. The collection will also enable entities whose authority has been revoked to undergo the process to resume operations, if they are not otherwise blocked from doing so. This multi-purpose form simplifies the process of gathering the information needed to process the entities’ requests in a timely manner, with the least amount of effort for all parties involved. The information reported to and collected by FMCSA is motor carrier identification information (such as legal names, DOT numbers, etc.), phone numbers, address changes, changes in ownership, management, or control of the company, reinstatement of operating authority information, and payment information. This ICR renewal has a program adjustment of an increase of 37,551 responses and 9,387 burden hours. The increase is due to the increase in the number of respondents who completed Form MCSA-5889 from 2020-2022.

The need and purpose of this information collection is to obtain information for Nurse Corps LRP applicants and participants. The information is used to consider an applicant for a Nurse Corps LRP contract award, and to monitor a participant’s compliance with the service requirements. Individuals must submit an application in order to participate in the program. The application asks for personal, professional, educational, and financial information required to determine the applicant's eligibility to participate in the Nurse Corps LRP. The semi-annual employment verification form asks for personal and employment information to determine if a participant is in compliance with the service requirements. Respondents include professional RNs or advanced practice RNs (i.e., nurse practitioners, certified registered nurse anesthetists, certified nurse-midwives, clinical nurse -0ecialists) who are interested in participating in the Nurse Corps LRP, and official representatives at their service sites.

The Centers for Medicare & Medicaid Services (CMS) is requesting approval of revisions to the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) Version 4.1 that will be effective on October 1, 2024. On April 7, 2023 the Centers for Medicare & Medicaid Services (CMS) issued a notice of proposed rulemaking (NPRM) (88 FR 20950) which proposes modifications to the collection of quality reporting data in the Inpatient Rehabilitation Facility Quality Reporting Program (IRF QRP). Specifically, CMS proposes to adopt two new measures and remove three measures from the IRF QRP. Mandatory Civil Rights Data Collection

Abstract

The collection, use, and reporting of education data is an integral component of the mission of the U.S. Department of Education (Department). The Department has collected civil rights data about the nation’s public schools via the Civil Rights Data Collection (CRDC) since 1968. As with previous CRDC collections, the purpose of the 2025–26 and 2027–28 CRDCs is to obtain vital data related to the civil rights laws’ requirement that public local educational agencies (LEA) and elementary and secondary schools provide equal educational opportunity. The Department has analyzed the uses of many data elements collected in the 2020–21 CRDC and sought advice from experts across the Department to refine, improve, and where appropriate, add or remove data elements from the collection. CRDC data definitions and metrics are consistent with other mandatory collections across the Department wherever possible. The Department seeks the Office of Management and Budget's approval under the Paperwork Reduction Act to collect from LEAs the elementary and secondary education data described in the sections of Attachment A. The Department requests that LEAs and other stakeholders review and comment on the proposed changes (detailed in Supporting Statement A, Attachments A-1, A-2, A-3, and A-4, and Attachment B).

National Communication System for Runaway and Homeless Youth Data Collection

Abstract

The Family and Youth Services Bureau’s (FYSB) Runaway and Homeless Youth Division has a legislative requirement to fund a National Communication System, which is currently operated by the National Runaway Safeline (NRS). The NRS provides information, referral services, crisis intervention, and prevention resources to vulnerable youth and families of youth at risk of running away and/or becoming homeless at no cost. When necessary, the NRS refers runaway and homeless youth to shelters, counseling, medical assistance, and other vital services. The NRS collects information from all contacts with youth and adults connecting with the NRS (i.e., parents, family members, legal guardians, service providers) on a voluntary basis to inform crisis services and develop an annual report on the information collected during calls, chats, emails, and forum posts from young people who reached out to the NRS’s crisis services. The Runaway and Homeless Youth Act (RHYA), as reauthorized by the Reconnecting Homeless Youth Act of 2008 (P.L.110-378) through FY 2013 and more recently reauthorized by the Juvenile Justice Reform Act through FY 2019, mandates the Secretary shall make grants for a national communication system to assist runaway and homeless youth in communicating with their families and with service providers. The NRS is required to have a system for collecting and analyzing data on calls, emails, chat, texts, and online messages received as well as other information, such as prevention resources, referrals, demographics, and visitors to the NRS website. The NRS must submit monthly and semi-annual reports that includes the following: • Number of calls received, answered, and missed. • Number of chats, emails, and texts received; number of chats, emails, and texts answered; and number of chats, emails, and texts that were missed and did not receive a response, in which the users are youth in crisis, runaway youth, and youth experiencing homelessness. • Number of parents, legal guardians, and service providers contacting the NRS and the type of resources, interventions, and technical support/assistance requested and provided. • Number and type of prevention materials disseminated to communities, especially to underserved populations. • Number and type of unique visitors to the NRS’ website. • Information on referrals provided and where youth were referred for services. • Information on the callers' or users' demographics and where they were located when contacting the NRS. • Information on the prevention materials developed and disseminated by the NRS. • Information and analysis of the latest trends and their impact on runaway prevention. The NRS uses two forms, one form to collect relevant information disclosed during calls, emails, and forum posts, and then a second form to collect information from chats. All data will be provided to FYSB in the aggregate and no personally identifiable information are collected. This nonsubstantive change request is in response to the Defending Women from Gender Ideology Extremism and Restoring Biological Truth to the Federal Government Executive Order (Defending Women EO). ACF has implemented changes to update gender collected data to denote sex as a biological variable in current approved information collection requests to comply with the directives. This request is specific to only these updates.

The National Assessment of Educational Progress (NAEP), conducted by the National Center for Education Statistics (NCES), is a federally authorized survey of student achievement at grades 4, 8, and 12 in various subject areas, such as mathematics, reading, writing, science, U.S. history, and civics. The National Assessment of Educational Progress Authorization Act (Pub. L. 107–279, title III, section 303) requires the assessment to collect data on specified student groups and characteristics, including information organized by race/ethnicity, sex, socio-economic status, disability, and limited English proficiency. It requires fair and accurate presentation of achievement data and permits the collection of background, noncognitive, or descriptive information that is related to academic achievement and aids in fair reporting of results. The intent of the law is to provide representative sample data on student achievement for the nation, the states, and subpopulations of students and to monitor progress over time. NAEP consists of two assessment programs: the NAEP long-term trend (LTT) assessment and the main NAEP assessment. The LTT assessments are given at the national level only and are administered to students at ages 9, 13, and 17 in a manner that is very different from that used for the main NAEP assessments. LTT reports mathematics and reading results that present trend data since the 1970s. In addition to the operational assessments, NAEP uses two other kinds of assessment activities: pilot assessments and special studies. Pilot assessments test items and procedures for future administrations of NAEP, while special studies (e.g., the Middle School Transcript Study (MSTS), and the High School Transcript Study (HSTS)) are opportunities for NAEP to investigate particular aspects of the assessment without impacting the reporting of the NAEP results.  This request is an Amendment to the initially approved NAEP 2026 Clearance Package (OMB# 1850-0928 v.36) to conduct NAEP in 2026, specifically: (1) Main NAEP operational assessments will include for grades 4 and 8 (first administration of the new frameworks for reading and mathematics), grade 8 (civics and U.S. history); in Puerto Rico, grades 4 and 8 mathematics will be the only subject assessed and will include the new framework; (2) Pilot testing in grades 4, 8, and 12 (reading and mathematics); in Puerto Rico, grades 4 and 8 mathematics will be the only subject assessed. Some documents in this package will be updated in Amendment #2, which will be posted for a separate 30-day public comment period following the 30-day public comment period for Amendment #1. These packages will contain all final materials to be used for the data collection in early 2026. As of April 2025, NCES’s assurances of confidentiality protections for NAEP 2026 have changed due to recent staffing changes at the Department of Education. NCES has removed the Foundations of Evidence-Based Policymaking Act of 2018, Title III, Part B, Confidential Information Protection (‘‘CIPSEA’’) as a confidentiality assurance. However, confidentiality assurances under the Education Sciences Reform Act of 2002 (ESRA) remain in effect.

This control number enables the BLM to collect information that is necessary to authorize and manage rights-of-way for solar and wind energy, oil and gas pipelines, and electric transmission lines with a capacity of 100 Kilovolts (kV) or more. The Bureau of Land Management (BLM) is requesting to revise this control number to reflect changes to the underlying regulations pertaining to renewable energy rents affecting solar and wind energy developments. This proposed rule would update the procedures governing the BLM’s right-of-way programs, focusing on one main topic that would support Unleashing American Energy (EO 14154). Respondents participating in the Medicare program use Supplemental to Form CMS-2552-10: Median Payer-Specific Negotiated Charge Data Worksheet to report payer-specific negotiated charge data pursuant to the CY 2026 OPPS Proposed Rule (CMS-1834-P) published July 17, 2025. This supplemental form advances the policy goal of increasing healthcare price transparency and promoting market-based approaches in Medicare FFS payments. The form requires hospitals to report, by MS-DRG, the weighted median of the payer-specific charges negotiated with Medicare Advantage organizations. The information reported on the worksheet enables CMS to calculate new IPPS MS-DRG relative weights beginning in FY 2029 and reinforces the administration's commitment to clear, accurate, and actionable healthcare pricing information.

Abstract

The IAF works to promote sustainable development in Latin America and the Caribbean by offering small investments directly to civil society organizations through funding actions, such as grants and cooperative agreements. By gathering perceptions from grantees on how the IAF works as a funder, the IAF is able to assess its performance and identify opportunities for improvements. The IAF seeks to work with a contractor to independently carry out this survey with IAF grantees. The contractor will use an online survey with a set of standardized questions focused primarily on grant processes, such as the approach to grant selection, the time lapse between selection and commitment, and reporting and evaluation. The contractor will also apply these standardized questions to other funders, thus providing the IAF with findings relative to that of other comparable organizations.

Coronavirus Capital Projects Fund Reporting

Abstract

Section 604 of the Social Security Act (the “Act”), as added by section 9901 of the American Rescue Plan Act of 2021, Pub. L. No. 117-2 (Mar. 11, 2021) authorized the Coronavirus Capital Projects Fund (“CPF”). The CPF provides $10 billion in funding for the Department of the Treasury (“Treasury”) to provide grant payments to States (defined to include the District of Columbia and Puerto Rico), seven territories and freely associated states (including the United States Virgin Islands, Guam, American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of the Marshall Islands, the Federated States of Micronesia, and the Republic of Palau), and Tribal governments to carry out critical capital projects directly enabling work, education, and health monitoring, including remote options, in response to the public health emergency with respect to the Coronavirus Disease (“COVID-19”). Section 604(b) of the Act prescribes that the $10 billion be allocated to eligible grant recipients according to a formula provided in the statute. Treasury has used this formula to calculate the CPF grant fund allocations for each eligible recipient and has posted these allocations on its website. In general, each State (including the District of Columbia and Puerto Rico) will receive between approximately $107 million and $500 million, each of the seven named territories and freely associated states will receive approximately $14 million, and each Tribal government will receive approximately $167,000. Treasury has previously received approval under OMB #1505-0274 for information collections related to applying for CPF grant awards (“Applications”) and for specifying the use of funds (“Grant Plans”) The current information collection will be used to solicit information related to quarterly project and expenditure reports and annual performance reports. Both information collections are described generally in the Compliance and Reporting Guidance. The Compliance and Reporting Guidance provides recipients information needed to fulfill their reporting requirements and compliance obligations. Treasury will also prepare an IT portal user guide with specific instructions on entering data into the reporting web-based portal.

45 CFR 303.7 - Provision of Services in Intergovernmental IV-D; Federally Approved Forms

Abstract

The federally-approved intergovernmental forms are used to facilitate interstate child support case processing when more than one state child support agency is involved, estimated to be in about 15-25% of child support cases. Use of these federal forms is required by 45 CFR 303.7(a)(4) and Section 311(b) of the Uniform Interstate Family Support Act (state law that states must adopt as a condition of receiving federal funding for child support). This information collection was first approved in 1988 and has evolved over time. The current set of forms will expire in February 2026. Currently, there are 13 intergovernmental forms each addressing different case processing actions that may be needed in a case. Some of the forms are for agency use only, while some may be completed by private individuals or entities involved in the case. On November 20, 2023, OMB approved OCSE’s request to make four of the intergovernmental forms available in Spanish: 1. Declaration in Support of Establishing Parentage 2. Uniform Support Petition On March 1, 2025, Executive Order 14224 was issued, designating English as the official language of the United States. In support of the President’s Executive Order 14224, OCSE requests to discontinue use of the four Intergovernmental forms currently available in Spanish. We are not requesting any changes to the English versions of the forms.
